Diabetes Specialist Nurses

The Diabetes Specialist Nurses work closely alongside the Consultants, and review Diabetic patients in the hospital environment.  They provide:

 

  • Specialist advice, GLP-1 and insulin initiation, crisis advice

  • Education on all aspects of diabetes for patients

  • Education for medical and nursing staff

  • Bridge the gap until community follow up is in place

 

They are based in Richmond House and cover both the Royal Gwent & St Woolos Hospital.

 

Each DSN has a special interest in various aspects of Diabetes, but collectively work as a team to provide support for both the Inpatient and Outpatient services.

 

They have a telephone referral service – call 01633 234270 and leave a voice mail message, stating Ward, Patients Name and CRN, reason for referral and estimated discharge date if known.

 

Daily insulin titration is undertaken by the junior medical team and Consultants, but where DSNs have reviewed the patient, an entry is made in the patient notes and treatment adjustments and alterations might be suggested based on their consultations with the patients and their families.
